Enroll by Mail/Fax

Complete an enrollment form in three simple steps and fax or mail it:

1. Download your enrollment form (pdf). To transfer credits earned at another high school to Keystone, download the transcript request form (pdf).
2. Select your courses from our course catalog and calculate your tuition. If you would like to pay over several months, download and complete the Keystone Tuition Payment Plan form (pdf).
